Apple have been working with the CDC, the White House Coronavirus Task Force and FEMA to produce a COVID-19 website and COVID-19 app. The tool helps individuals find official guidance based on symptoms, exposure and other risk factors. Anyone can use it and it does not require a sign in or association with an Apple ID. The app is targeted at individuals in the US but the guidance may still be helpful. You can cross reference and stay up to date with the official UK guidance here.
